By checking, You will agree To participate annually in the program, you must register as a member of the Active Adult Center of Mississauga  and as a member of the Alce For a Full Life program. The total value is canceled directly to the Active Adult Center of Mississauga.

Informed Consent:

 By personal initiative, I participate in the program and release Alce Art and Health and Active Adult Center of Mississauga from any liability in case of an accident or injury during program activities.

I authorize the taking of photographs to promote the activities of the program in printed and audiovisual media.
